Specialist Support Coordination (Level 3)
All staff delivering this service hold allied health qualifications, ensuring that coordination is informed by clinical understanding and experience.
Our role extends well beyond arranging referrals or monitoring service agreements. We work alongside clients, families and services to coordinate care where multiple systems are involved, including mental health, forensic, alcohol and other drug services, housing, justice, disability services and other mainstream providers.
We support teams to work collaboratively, reduce siloed decision‑making, and stay aligned with what is achievable within NDIS funding, while also thinking creatively about how to build cohesion across services. Our aim is to ensure that each client’s supports are responsive, coordinated and oriented toward outcomes that genuinely improve their daily lives and long‑term wellbeing.
